93 patients were reviewed, who had received bilateral subtotal thyroidectomy for hyperthyroidism from 1986 to 1990 at Kwang Ju Christian Hospital and had been followed up for at least 6 months or more.
The results were as follows:
1) Male to female ratio was 1: 8.3 and highest incidence in the second and third decade occupying 72.1%, 63 cases out of 93.
2) Less than 1 year duration of symptoms and signs of hyperthyroidism was most frequent with 366%, 34 cases out of 93.
3) In thyroid function test, the mean value of T3 and T4 was 489.8ng/dl and 21.35 ug/dl and the mean value of 131I uptake was 66.95%.
4) In preoperative preparation, prophylthiouracil, lugol solution were given to 54 cases(58.1%) and prophylthiouracil or methimazol, lugol solution and propranolol were given to 23 cases(24.7%).
5) In pathologic classification, 91 cases(97.8%) were graves' disease and 2 of them were combined occult papillary cancer, 1 cases is multiple toxic goiter, 1 cases is Hashimoto's thyroiditis.
6) Overall rate of euthyroidism, hyperthyroidism and hypothyroidism bared on serum T3, T4 and TSH followed up for at least 6 months or more were 91.3%, 8.3% and 1.1%.
7) Postoperative complications were transient hypocalemia in 11 cases(11.8%), prologed hypocalemia sustaining more than 6 months in 2 cases(2.1%), recurrence in 8 cases(8.6%), hypothyroidism in 1 cases(1.1%), wound infection in 5 cases(5.4%),
hematoma
in 1 cases(1.1%), thyroid crisis in 1 cases(1.1%), but there was neither recurrent nerve injury nor postoperative death.
8) The mean weight remaining thyroid tissue was 7.1gm. In 8 patients of postoperative recurrent hyperthyroidism mean weight remaining thyroid tissue was 8.25 gm and according the degree of lymphocyte infiltration in thyroid tissue, there was 3
cases in
moderate, 1 case in mild, 4 cases in no lumphocyte infilatration.
9) In patients of postoperative hypothyroidism, the weight remaining thyroid tissue was 6.0 gm with moderate lymphocyte infiltration.
